Call a participating chiropractor to set up an appointment. Be sure to tell the office that you are a "UHS Chiropractic" Member.
If your chiropractor is not currently participating in our program, please give the Member Service representative his/her name, address and telephone number and our provider relations department will contact the chiropractic office about joining our growing network of professionals.
